Long Term Condition Reviews

We have changed the way we manage our long term condition reviews. We are aligning reviews with patient’s month of birth and sending a text invite containing a link to book an appointment. If you do not have a mobile, you will receive a letter.

If you have a long term condition, you should expect your invite to book your review, the month before your month of birth.

If you have already had your review in the last few months and receive an invite to book, please do not ignore this invitation and book in as requested. This will ensure your review date is correct for next year.

Please ensure we have your up-to-date mobile number and you have consented to receiving text messages.
