If you have been advised by the surgery to submit a contraceptive pill review please use this form. When you are taking the oral contraceptive pill you need to have a review at least every 12 months. You may be due a review sooner if you have recently started or changed your contraceptive pill.
We do need to know your weight and blood pressure. You can check these using home devices or they can be done at most gyms and some pharmacies. If you are still unable to access any of these, you will need to book a face to face appointment with a nurse.
