Please check that your home address falls within the boundaries of our Practice area before completing a registration form.
The area covered by the Practice is shown on the map. It includes Oakham and surrounding villages with approximately a 6 mile radius. When registering please check with the Reception Staff if your village in included in our Practice boundary.
Please note that if you are an existing patient and move home outside of the Practice boundary, you may be asked to register with another Practice.
You can register as a new patient using our online registration form (this includes completing a GMS1 form and a New Patient Questionnaire). This will be sent electronically to the Practice; however, you will also need to visit the Practice with Proof of Identification before the registration process can be completed.
If you would prefer to visit the Practice, please bring your medical card (if you have this) and 2 forms of Identification (ID) with you. The Reception staff will ask you to complete a GMS1 form and a New Patient Questionnaire. The forms are available at the Practice or can be downloaded from the links below.
The Receptionist will then book you an appointment with the Practice Nurse for a New Patient Medical. We recommend all new patients to undertake this important medical as it gives us the opportunity to review your medical history with you to enable planning your future health care.
New registrations can be dealt with at any time whilst the Practice is open. It is usually quieter after 11am and it is advisable to try and attend after this time.
