Vitrucare is an online Long Term Conditions self-management tool accessible via PC/Laptop, Tablet or Smartphone device (internet access required).

If you have Diabetes, Hypertension or COPD, you can qualify to create an account for Vitrucare which shares information on how you can manage your Long Term Condition.

It also allows you to record your weight or BP and other condition related information this is then shared with us so that we can review your progress during your next Chronic Disease Management Annual Review appointment with our Nursing Team.

We will also use the information you send us to understand how you are progressing through the year and send you a personalised message to confirm your progress.

We are looking to pilot this with our Type 2 Diabetic Patients within the first phase, so if you have Type 2 Diabetes and would like to learn how you can better manage your condition and share your blood sugar levels with us, then please complete the form below to register.

If you are Hypertensive or COPD, you can still sign up and share your readings with us, however the nursing team will review your data from Vitrucare in the new year.

Once we have received your form, we will register you on Vitrucare and send you an email invitation for you to create your account.

Vitrucare is an interactive app which links into your medical records, it supports self-management of your Long Term Conditions (LTC) i.e Diabetes, Asthma, COPD. If you have a LTC, you can qualify to receive access to this app.

    Disclaimer: If you agree to Oakham Medical Practice registering you with Vitrucare, please note the following:

    • Your demographic details will be held with Vitrucare as you register with the vitrucare app.
    • If at any time you would like to opt out of the above services, please make a request in writing to the Practice and you will be opted out as soon as we have the opportunity to process your request. You may also like to include your reason for opting out, to help us review and improve the service in future.
    • Whilst the Practice will regularly check your contact information with you, please be aware that the onus of keeping your contact details current with the practice rests with you.
    • Once you have registered with the Vitrucare app, please explore the app and options available to you to manage your individual conditions - we will activate your LTC self-care package to enable this. UNLESS you are contacted by a clinician directly, Oakham Medical Practice will not be looking at your details or interacting with you via this medium until you are advised regarding collaborative working at your next annual review. We are aiming to provide a staged engagement process over the next 12 months to coincide with LTC annual reviews but did not want to prevent patients enjoying this facility before they are contacted or on the date of their annual review.
